This form is filled out by someone who has been involved in a motor vehicle accident. If the patient is also new to BCM they will be required to fill out the new patient forms for Chiropractic (Consultation Admittance, Health History, Systems Review) and Massage (Massage Intake) in addition to the AB-1. There is a lot of paper work on the first motor vehicle accident visit, please come to your appointment 20 minutes early to allow time for completion or complete at home and bring it in. The time to complete the paper work is not included in your visit with the Chiropractor.
